The First Report of the Chronic Disease Management Prevention Programme in General Practice

Over 155,000 people at risk of heart disease or diabetes have been enrolled onto the HSE’s Chronic Disease Management Prevention Programme in its first two years, helping patients to manage their health and reduce their risk of hospitalisation.
