This Report relates to an Independent System Analysis Review (SAR) conducted into the systems of care provided to Patient B following their admission to Hospital 1, on the 26th August 2021 with a head injury (traumatic brain injury) to the transfer of Patient B’s care from Hospital 1 to Hospital 2 on the 29th August 2021.
