To ask the Minister for Health the number of deaths by suicide that occurred within seven days, 30 days and 90 days of discharge from an acute psychiatric inpatient unit in each of the past five years and to date in 2026, by RHA; the number of serious incident reviews carried out in each case; and if she will make a statement on the matter.
