To ask the Minister for Health the dates at which CHI, the HSE and HIQA first became aware that one field safety notice (Further to HIQA identifying that CHI had documented communication with families in relation to three of four MAGEC rod field safety notices issued between 2020 and 2021, when CHI, the HSE and HIQA found out that one field safety notice had not been recorded as communicated to parents?) had not been recorded as communicated to parents; the actions subsequently taken; if all affected families were later identified and informed; her views on the governance, disclosure and patient safety issues arising from this; and if she will make a statement on the matter.
