To ask the Minister for Health whether Government is satisfied there is effective and consistently applied clinical governance oversight across HSE South West; whether identified clinical leadership gaps have been independently reviewed; whether vacancies, fragmented accountability or weak leadership structures have affected patient safety, service quality or risk management; the assurance that will be given that clinical governance failures are not contributing to repeated performance and safety concerns; and if she will make a statement on the matter.
