Learning from Practice - Demand Management Across the Patient Pathway

This report brings together 26 planned care case studies across the four sustainable care group specialties: dermatology, ophthalmology, ENT and plastic surgery, with representation from each of the six health regions. The case studies selected highlight practical reform and innovation in planned care across the patient pathway and reflect the integrated work of multidisciplinary teams across acute hospitals and community services. The report illustrates how practical action improves patient flow, patient experience, and supported access to the right care at the right time.
