Community Care Record


The HSE Community Care Record will provide a digital system for managing patient information in all community services. It is an important part of our digital healthcare system and a step towards HSE One Health Record.

The One Health Record will contain all patient health information in one secure, shared system.

Benefits of Community Care Record

Community health services have relied on paper or local systems. This can cause delays, duplication and frustration for staff and people using the services. Important information is not easy to share and it’s hard for staff to get a full picture of someone’s care.

Community Care Record will help community services staff manage referrals, appointments, documentation and caseloads digitally.  This will have a positive impact on healthcare services overall.

For patients and service users, this means:

  • care will be more consistent across services
  • staff will have easier access to clear, up-to-date information
  • there will be fewer delays in care
  • community appointments can be seen in the HSE Health App

For staff, this means:

  • safer, simpler workflows for everyday tasks such as appointments and referrals
  • less time spent on paperwork or repeating tasks
  • a clear, coordinated approach across services and regions

Launching Community Care Record

We are currently developing Community Care Record and planning its roll-out. So far, we have held many workshops, mapped clinical workflows and tested digital prototypes.

This has helped us to design a system with staff and patients' best interests in mind.

National and regional teams will work on setting up the system and making sure that staff are trained to use it correctly.  There will also be practice runs to ensure a smooth transition.

Single Point of Access (SPOA)

Single Point of Access (SPOA) is the first major feature of Community Care Record that will go live in 2026. It will provide a standardised digital entry point for referrals to community services.

SPOA replaces fragmented referral processes with a single digital workflow. It covers referral, triage and waiting list management. More functions will be made available as the system rolls out.

At first, staff will be able to create a single record for each child that can be used across all community services. Staff will also be able to manage referrals, organise appointments and maintain communication with a child's family as they move through the community care pathway.

Geographical roll-out

We plan to start rolling out Community Care Record on a regional basis. HSE Midwest, alongside a number of Specialist Palliative Care sites including Milford Care Centre and Our Lady’s Hospice and Care Services, will be first. This is due to go live during Q3 and Q4 2026. Full implementation is expected by the end of 2027.

The remaining regions will launch in this order:

  • HSE South West (all services)
  • HSE Dublin and North East (Child and Adolescent Mental Health Services & Specialist Palliative Care only)
  • HSE Dublin and South East
  • HSE Dublin and Midlands
  • HSE West and North West
