Preparations are well underway for the HSE Community Care Record, a digital system that will support how patient information is managed across all community services.

The first major feature of Community Care Record will go live in June 2026, providing early functionality to support the delivery of the Single Point of Access (SPOA) across community paediatric services. The roll-out will then begin regionally.

With a phased rollout starting in Q3 2026, the focus is now on ensuring regional readiness and supporting staff through this change. From Frequently Asked Questions to helpful resources and timelines, this link can help your understanding of the programme.

In recent weeks, we held a series of in person Regional Service Process Mapping workshops across HSE Mid West and selected Specialist Palliative Care sites, including Milford Care Centre and Our Lady’s Hospice and Care Services, which will be among the first to implement the system. We will be running further workshops with each region in line with their deployment timelines.

What are the Regional Service Process Mapping workshops?

These workshops, also known as Regional Service Blueprints, brought together staff from different services to map out how work is currently carried out in practice.

They built on previously agreed national workflows for the Community Care Record programme. During the sessions, teams explored local variations, key steps, dependencies, and challenges. This helps make sure local needs are understood. while staying aligned with the national system design.

The workshops also looked at how ways of working will change with a new digital system. Staff identified what they may need to stop, start or continue doing to support this change. This is important, as successful adoption depends not only on the technology, but also on how people use it to support their day-to-day work.

In these workshops, we produced process maps. A process map (or workflow) is a simple visual way to show the steps involved in delivering a service. It helps teams see what works well, where improvements can be made, and how processes can be streamlined.

Why are these workshops important?

These workshops gave staff time and space to review and improve how services work today. The aim is to support clear, consistent processes that align with new digital ways of working.

They also helped teams prepare for the future. Staff could ask questions, share concerns, and learn more about how the Community Care Record will support their work.

What are the outputs?

The workshops produced detailed process maps for each Mid West service within the scope of the Community Care Record, along with valuable insights from staff.

These outputs are now being used to support rollout and readiness activities, including:

  • change management
  • communications
  • system configuration and testing
  • training

What can we expect next?

As we already mentioned, Community Care Record will be introduced in phases. You can view the timeline below:

The planned order for rollout is:

  • HSE Mid West, along with several Specialist Palliative Care sites, including Milford Care Centre and Our Lady’s Hospice and Care Services
  • HSE South West (all services) and HSE Dublin and North East (Child and Adolescent Mental Health Services and Specialist Palliative Care only)
  • HSE Dublin and South East
  • HSE Dublin and Midlands
  • HSE West and North West
