Location
This is an appointment to the hub for the Integrated Care Programme for Chronic Disease, IHA KWW (Initial Base: St Mary’s Community Services, Craddockstown Road, Naas, Co. Kildare) by the Health Service Executive. The commitment for this post will be for 35 hours per week to work within the associated community-based ambulatory care hub. It is noted that the post must be congruent with the requirements of, and facilitate implementation of Sláintecare, and the HSE’s National Framework for the Integrated Prevention and Management of Chronic Respiratory Disease, the National Clinical Programme for Respiratory and the Enhanced Community Care Business Case, including commitment to deliver the relevant performance outcomes.
